$8,000 Sign On Bonus! RN Care Manager Job
Responsibilities:
Manages caseload of Pace Program members Responsible for clinical assessments and home visits Working collaboratively to develop, member centered care plans, provide on-going care coordination,member education, quality assurance and monitoring of services.
Completes all required documentation and charting and assists in the development and maintenance of member records.
Regular local travel - requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
RN Care Manager Job Requirements:
Education:
Associates degree in Nursing Required, Bachelors Degree preferred.
Licensure:
Wisconsin Licensed Registered Nurse required Eligibility for long-term care functional screener certification required WI Drivers License
Experience:
2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity required 2 years of work in community nursing preferred RN Care Manager Job Schedule:
